About the Job
We are seeking a highly experienced and versatile Patient Navigator to support a specialized breast health practice. The Patient Navigator plays a key role in ensuring seamless access to care by managing and triaging new patient referrals, coordinating appointments, and supporting clinical workflows. This role requires prior clinical experience to effectively interpret medical referrals, prioritize high-risk cases, and coordinate imaging and follow-up care for established patients.

Key Responsibilities:
Referral Triage & Scheduling:
  • Review, sort, and process all inbound referrals for new patients.
  • Interpret clinical documentation to determine the appropriate specialty or provider.
  • Identify urgent or high-risk cases and ensure they are prioritized for timely appointments.
  • Communicate with referring providers to clarify incomplete or unclear referrals.

Patient Scheduling:
  • Schedule initial consultations for new patients, based on clinical urgency.
  • Arrange follow-up visits and continuity of care for established patients.
  • Schedule imaging studies (MRI, Biopsy, Mammogram, US.) and other diagnostics as ordered by providers.
  • Coordinate appointment times with the provider’s availability and department resources.

Care Coordination & Patient Communication:
  • Serve as the point of contact for patients navigating the healthcare system.
  • Educate patients on the next steps in their care journey, including imaging prep and follow-up expectations.
  • Communicate imaging results to patients and provide instructions on next steps in the care process

Administrative & Clinical Support:
  • Accurately document patient information and communications in the Electronic Health Record (EHR).
  • Collaborate with clinical and administrative teams to streamline care coordination.
  • Monitor pending referrals and follow up on incomplete scheduling tasks or outstanding imaging.

Medical Scribing:
  • Accurately document patient encounters in real time during clinic visits.
  • Assist providers with chart completion, orders, and follow-ups.
  • Maintain thorough and compliant medical records.

Qualifications:
Required:
  • Minimum of 1–2 years clinical experience in a healthcare setting (e.g., LPN, medical assistant, nursing assistant).
  • Working knowledge of medical terminology, referrals, and diagnostic imaging procedures.
  • Proficiency with Electronic Health Records (EHRs) and scheduling systems.

Preferred:
  • as an LPN, Medical Assistant (CMA), or other relevant clinical experience.
  • Experience in care coordination, patient navigation, or referral management.
  • Experience in breast health, oncology, or women’s health .

Skills:
  • Strong attention to detail and organizational skills.
  • Effective communication and interpersonal abilities with patients and medical staff.
  • Ability to manage multiple priorities and work in a fast-paced environment.
